Effective March 1, we will begin collecting co-payments, co-insurances and deductibles at the time of your visit. Also, you must have your insurance card and photo ID at each visit, or we will not be able to see you. If you have questions, please call (614) 898-7546 to speak with a billing associate or manager prior to your appointment.
Any charges not covered by your health care plan will be billed to you after your appointment. You are responsible for all costs not covered by insurance. For out-of-pocket expenses, including deductibles, co-insurance and co-payments, we accept Visa, MasterCard and Discover credit cards, as well as personal checks.

While we do participate in the majority of major insurance programs, there are some plans that fall under these well-recognized company names that we are not providers for. Below is a partial listing of plans COSC DOES NOT participate in. Although not all are listed, please read your card carefully. Any plan with the words "catastrophic, bronze, silver, gold, or platinum" in the name are also ones we are not contracted with. We ARE NOT providers for any of the government exchange products, whether they be at the state or federal level.
